What to bring on the day of your child's procedure

  • Insurance cards, medical cards and your child’s Social Security number
  • A copy of your child’s immunization record
  • Proof of guardianship (for court-appointed guardians)
  • A complete medication list
  • Containers for eyeglasses, contact lenses and hearing aids (leave valuables such as jewelry at home)
  • Your child’s favorite toy, blanket, or comfort item
  • Special bottle, pacifier or sippy cup
  • Loose-fitting comfortable clothing for your child to wear after the procedure
  • Something to occupy your time during your child’s procedure, for instance, a book, magazine, knitting project or laptop (wi-fi access available)
